If you do not know your insurance coverage policy, please contact your insurance carrier to find out if service at the Carol Milgard Center are within your plan description.
If you have no insurance, you may pay privately. A sliding fee scale is available for our patients who meet eligibility criteria. In some cases there will be no fees applied for services received. In any circumstance, please do not delay your mammogram because you are concerned about payment. Talk to us!
Discounts are available to private payers.
Payment Terms
Carol Milgard Breast Center accepts all patients and bills most insurance plans including Medicare and Medicaid. When you present your current insurance card or complete insurance information, Carol Milgard Breast Center will bill your insurance carrier. If your insurance card is not presented at the time of service, your bill will be mailed to you.
Your financial responsibility for your examination or procedure will depend on the type of insurance plan you have and the individual contract Carol Milgard Breast Center has with your insurance company. If you have questions about your insurance benefits (coverage), you should contact your insurance carrier directly. Please understand that billing does not guarantee eligibility or payment. Patients are responsible for all co-payments and deductibles.
