Patient referral made easy
There are two ways to refer a patient to the Carol Milgard Breast Center. You can call our patient referral desk, which is open from 8 a.m. to 5 p.m., Monday thru Friday, or you can print the community referral form. Simply fill out the form and fax it to us.
