Contracted Insurance Providers

We are not contracted with Kaiser Core or Kaiser Medicare Advantage HMO plans. If you have either of these plans you must have an authorization from your insurance company prior to the exam. We are contracted with Lifewise Primary but are out of network. Please verify if you have out-of-network benefits before scheduling your exam.
