Program Requirements
Each church agrees to:
- Participate in FaithHealth in Action (FHIA)
- Assemble a health ministry team with an assigned primary health ministry lead
- Fill out the online contact form for the program. (English | Spanish)
- Attend required orientation and complete online health trainings
- Conduct a Pre and Post-Year survey of their congregation
- Create and implement a health ministry Action Plan
- Provide requested statistics and reports
- Use all funds for activities described in the Action Plan
The grant award will be disbursed as follows:
First Sponsorship Installment of $1750 after:
- Receipt of completed surveys
- Completion of online health trainings
- Submission of completed Action Plan
Second Sponsorship Installment of $1750 after:
- Receipt of Event Activities Sheets (English | Spanish)
- Attendance and participation at MidYear Celebration
Application for two micro grants of up to $500 each to assist in the implementation of a health fair, screening event or health ministry presentation will be available after:
- Submission of completed Action Plan
